LPN NURSING STATUS: Part-time and Full-Time
LPN SHIFT & SCHEDULE:
- 1st Shift, 6:30AM - 3:00PM, including every other weekend
- 2nd Shift, 2:30PM - 11:00PM, including every other weekend
LPN NURSING BASE WAGE: Up to $29.50/hour + $3000 Sign On Bonus!
- 2nd SHIFT STARTING BASE WAGE $25/HOUR PLUS ADDITIONAL $3/HOUR 2nd SHIFT DIFFERENTIAL
- 1st SHIFT STARTING BASE WAGE $28/HOUR
- WEEKEND SHIFT EARNS AN ADDITIONAL $1.50/HOUR SHIFT DIFFERENTIAL

LPN NURSING ROLES & RESPONSIBILITIES:
- Assist in the training of staff.
- Occasional flexibility may be needed.
- Perform evaluations.
- Perform office tasks for the Director of Healthcare when the Director of Healthcare is not available.
- Manage resident’s TB screening and supporting documentation.
- Update services plans with PT, OT, VNA, Home Health Services, Hospice, etc.
- Assist Director of Healthcare gathering data to create and update service plans.
- Present in-services for staff, as requested.
- Assure quality customer service and performance is achieved.
- Participate in the Interdisciplinary Team meetings.
- Assist in transfer/discharge arrangements, as requested.
- Assist Director of Healthcare in notifying Doctor and family of any health concerns, as requested.
- Assist Director of Healthcare in performing and/or supervising monthly vital signs and weight checks.
- Maintain first aid kits and blood spill kits.
- Assist the Director of Healthcare in overseeing all medication reminders and management.
- Assist in proper disposal of hazardous materials.
- Assist in maintaining inventory of nursing equipment and supplies and perform annual inventory of all items related to health care.
- Use personal protective equipment, as required, and monitor staff for appropriate usage.
- Document resident’s health concerns.
- Attend all mandatory meetings.
- Be a part of the safety team and committees as assigned.
- Attend bi-weekly staff in-services and meetings.
- Be a team player.
- Assure that appropriate medical information is relayed between shifts.
- Uphold confidentiality and Health Insurance and Portability and Accountability Act (HIPAA) regulations.
- Follow the Communication Log procedure to communicate with staff members, and read daily.
- Report any maintenance needs by using work orders.
- Document employee, self, and resident incidents by using designated incident report forms.
- Complies with all federal, state and local laws and regulations.
- Comply with all the Community policies and procedures.
- Comply with the Community’s Employee Handbook.
- Supervise and perform duties of caregivers, when needed.
- All duties as assigned.
